Save the file when it is completed and email to durhamsoccerleague@yahoo.ca.
Registration cost is $250 per team.
Please make cheques payable to the "Durham Soccer League," and mail to:

TRAVEL PERMITS:
This is an OSA approved tournament. Each team must be OSA affiliated and, if travelling outside of your district, provide the appropriate travel permit . Travel permits can be filled out through the OSA website.
REGISTRATION:
Saturday, July 24th, 2010
8:00am to 10:00am
Scugog Fields, Port Perry.
Please bring a printed paper copy of your roster with OSA player books, and travel permit (if not mailed with cheque) when registering in person before the tournament.
